Community Health Worker
Full-Time

The community health worker will work within the community and healthcare settings to empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to Primary Care and community-based social supports. This position reports to the director of caregiver Services and works closely with New York Presbyterian Hospital .
Qualifications
Must hold a high school diploma or GED
Two years of work in a community health role or comparable experience
Comfortable conducting home visits and visiting families in the hospital or community settings
Experience facilitating and presenting workshops and trainings
Demonstrated ability to work collaboratively and work with families experiencing exceptional challenges
Ability to use Microsoft Work and Outlook
Bilingual English and Cantonese, Mandarin, or Spanish
Responsibilities
Reporting to and working closely with the HMH contact and NYP program manager
Managing designated caseload and meeting stated program goals
Conducting home and hospital visits
Offering practice-based support and information sessions across a variety of locations
Providing virtual support to patients and assists with education around virtual Healthcare
Collaborating with Healthcare team throughout the patient-centered medical home and Hospital experience
Completing appropriate documentation of Assessments and educational sessions at program intervals using determined collection method while maintaining an organized filing system and submitting all paperwork/ documentation to program staff by the deadline
Facilitating two community-based workshops per year and conducting community-wide outreach via event planning, workshops, and health fairs
Attend all required staff meetings and trainings
Compensation: $26 per hour
Hours: Monday to Friday, 35 hours per week
