Position Title : Community Health Worker (CHW)

Supervisor : Asst. Director of Population Health

Job Type: Full-Time (37.5 hours weekly)

Department : Population Health

Schedule Shift : Monday- Friday 9:00am-5:30pm and weekends as needed.

FLSA : Exempt

Salary : $50,000 - $60,000

Bronx Community Health Network (BCHN): was founded in 1996 to ensure that the socio-economically depressed and medically underserved residents of the Bronx have access to high-quality and comprehensive healthcare services, Bronx Community Health Network (BCHN) is a not-for-profit, community-based organization and Federally Qualified Health Center. The organization provides a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents. BCHN’s service area includes the entire borough of the Bronx, including all 25 zip codes.

BCHN mission : To improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured persons; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services.

About the Role : The Community Health Workers provides Case Management Services to individuals to help them gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community. These services are coordinated with internal and external providers to help individuals achieve personalized goals. CHW will work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports.

• Engage clients in ways that create trust and rapport with them and their families
• Create a non-judgmental atmosphere in interactions with clients and their families
• Identify key community stakeholders and organizations
• Work closely with schools and healthcare providers to ensure persons served have comprehensive and coordinated care plans
• Encourage clients to engage in behavior change, utilize needed services and advocate for themselves
• Provide advocacy, education and outreach on behalf of vulnerable youth and young adults
• Maintain and utilize a list of community resources to meet health care and social service needs of clients (health care, social services, and additional support)
• Participate in on-going meetings and conferences regarding issues that influence CHW work and ability to function effectively as a CHW
• Identify and document community needs and assets so that BCHN can evaluate community issues and plan for appropriate response
• Completes required documentation associated with assessments and educational sessions using approved collection method. Use of documentation and reporting systems to maintain record of engagements with clients and partners
• Make social services and other referrals to address social determinants of health. Share health information and written materials with clients, families and at health centers
• Performs other duties, as assigned.

Qualifications and Skills

• Bachelor's Degree (Required)
• Master’s Degree (Preferred)
• 2+ years of CHW experience or comparable education/training experience preferred
• CHW Certificate (Preferred)
• Experience facilitating workshops and providing one-on-one health support and information is preferred

• Familiarity with local resources that could help individuals and families.
• Problem – solving and critical-thinking skills.
• Ability to collaborate and work well as part of a team.
