Job Description

The Hospice Admissions Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Admissions Coordinator processes patient referrals, coordinates the admission of patients, and enters patient admission data after all requirements of hospice have been met. Position Specific Responsibilities include attending and participating in staff meetings, educational programs and community events, communicating identified needs and potential solutions to supervisor, receiving and processing referrals from physicians, patients, families, friends, and community, contacting physicians to secure an order for admission, contacting insurance companies for pre-certification authorization, scheduling appointments with patients and families for informed consents and the RN assessment visit, explaining hospice services and benefits to patients, family members, and significant others, informing appropriate team members of time of appointments, completing admission process according to agency/corporate policy, ensuring proper notification of admission to assigned interdisciplinary team (IDT) and ancillary providers, entering admission data into a third-party system, ensuring patient chart documentation is signed and dated and turning chart over to Team Coordinator, marketing referral sources, contacting long term care staff, hospital staff, and physicians to promote hospice services, providing back-up support for Hospice Care Consultant, documenting marketing contacts, and performing other duties as assigned. Education and/or Experience required includes a high school diploma or GED, with a bachelor's degree preferred, two to three years of related experience and/or training strongly preferred, experience communicating with healthcare personnel highly preferred, and experience in insurance and Medicare/Medicaid billing and reimbursement preferred. Skills required include mathematical skills, language skills, and other abilities such as understanding, reading, writing, and speaking English and embracing hospice philosophy. Physical demands include regular standing, walking, manual dexterity, ability to lift and move objects up to 25 pounds, and visual acuity requirements.
