JOB DESCRIPTION

Position: Housing Outreach Navigator – El Cajon

Classification Non - exempt

Supervised By: Housing Manager

Job Summary: The Housing Outreach Navigator performs street-based outreach to engage and builds rapport with unsheltered individuals and families experiencing homelessness in El Cajon. The Housing Outreach Navigator will identify highly motivated clients, connect clients to resources, stabilize clients in scattered site emergency shelter, and identify permanent housing solutions.

Duties and Responsibilities:

  1. Conduct street-based outreach to develop and build rapport with clients.
  2. Coordinate intake and needs assessments for all clients.
  3. Work with clients to develop housing stability plans that address barriers to obtaining housing.
  4. Provide intensive case management to support housing stability goals including advocacy, assistance with procuring documentation and services such as identification or disability income/social security.
  5. Maintaining expertise of pertinent community services and resources to effectively provide information, referrals, linkages, and advocacy that assist clients in obtaining permanent housing.
  6. Identifying appropriate permanent housing options for clients based on their needs, aiding with housing ocation and application, and advocating for clients with prospective landlords.
  7. Developing relationships within the community, including local businesses, landlords, and other service providers, to identify new and existing opportunities and build relationships to better assist clients in accessing resources, employment services and housing.
  8. Meet with established clients on a consistent basis to review, evaluate, and support goal completion.
  9. Coordinates to identify appropriate housing for clients.
  10. Makes referrals to community resources as needed.
  11. Works with clients on obtaining benefits and/or increase income.
  12. Maintain client files and records.
  13. Complete client case notes.
  14. Participate in staff meetings, case conferencing, and training as assigned by supervisor.
  15. Other duties as assigned.

Qualifications:

Education : A Bachelor’s degree or equivalent in social work, social services, and/or related human services field preferred. Knowledge : Knowledge of community resources and evidence-based practices. Skills: Must have effective communication skills and be willing to work as a team player. Abilities: Demonstrated ability to be culturally sensitive and respect diversity. Essential personal attributes include: stability, sensitivity, flexibility, patience, discretion, warm and friendly disposition, and non-judgmental attitude. Other Requirements: Must have a valid California driver’s license, a safe driving record, automobile insurance, a reliable vehicle and be willing to use it on the job.

Physical Demands:

  • Reading and typing on a computer frequently.
  • Sitting for long periods of time.
  • Traveling in car to meet with clients or funder.

Work Environment:

  • Office space may be shared with frequent telephone and visitor contacts.
  • The noise level fluctuates from quiet to noisy.
