Exciting Opportunity to join and represent Commonwise in a Brand-New, Luxury Style, All Inclusive Independent Living Community.

Commonwise Home Care is seeking a Registered Nurse to serve in a role we call Independent Living Nurse Advocate. The Nurse Advocate will work from the Main Street office in the community, building and maintaining relationships with residents who may need companionship or personal care services. For Community Residents in need of services, the Nurse Advocate will initiate and oversee the delivery of care services through onsite professional Commonwise Caregivers. and serves as an ambassador for the organization and resource to the aging adults living in the community.

Expected Hours:

Monday through Friday 7AM - 4PM

Key Responsibilities  

Care in Private Residences 

In-Home Care Assessments: 

• Advocate alongside Community Residents for the overall wellness at home 

• For those seeking assistance:

• Assess and Plan with member or representative care needs in a structure care plan

• Assess, Plan Medication Management for participating Community Residents 

Care Management: 

• Prepare Commonwise Caregivers to implement the care plan 

• Engage and manage Caregivers throughout the relationship 

• Support Community Residents in ongoing, long term care relationships

Community Engagement:

• Build relationships within the Community Residents 

• Host periodic educational workshops and events for members of the independent living community

• Explain our services and business value proposition to interested community resident members 

Candidate Requirements : 

• Highly value Commonwise’s Professional Caregivers, supporting their engagement and retention 

• Prioritizes Building Relationships as a Commonwise brand cornerstone 

• Passionate for advocacy, particularly for the aging

• Enjoys a range of conversation from small talk, careful navigation of current events, and the in-depth aging needs 

• Skilled at presenting information in an engaging and informative manner. 

• Passionate about making a positive impact on the lives of older adults and their families. 

• Knowledgeable about local resources (or willing to seek out and learn quickly) and services related to aging 

• Registered Nurse 

Ideal Candidate: 

• Experience in the aging space, long-term care, Geriatrics, or significant Medical-Surgical
