Colonial Heights Rehabilitation & Nursing Center is seeking a full time Social Worker Assistant. The ideal candidate must be devoted and compassionate about their chosen career. We are seeking a self-motivated individual who will strive to create and maintain a warm atmosphere and a positive experience for residents, visitors, and staff. Maintaining excellent working relationships is also important. Leadership abilities and effective communication skills are necessary for this position.

Qualifications for a Social Worker :

• Bachelor's degree in social work or similar degree from accredited college or university.
• Sufficient experience in order to perform daily tasks with an understanding of the duties involved.
• At least 1 year social work experience at a long-term care facility.

Responsibilities of Social Worker:

• Provides considerate respectful care focused upon the Residents' individual needs.
• Assists the Resident in the exercise of his/her rights and informs the Resident of any responsibility's incumbent upon him/her in the exercise of those rights.
• Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the Residents cared for.
• Demonstrates an understanding of the range of care needed based on the age of the Residents cared for.
• Demonstrates knowledge of growth, development and/or adult health throughout the life span as appropriate to the job performed.
• Successfully completes life-span education as appropriate.

Skills of Social Worker : All qualified applicants should possess the following qualities:

• Excellent communication skills both written and verbal.
• Ability to respect all members of the healthcare team
• Strong documentation skills
• Solid work history with excellent attendance and punctuality patience, understanding and compassion for those requiring our care
