Job Summary – RN Case Manager / Care Coordination
Shift: 8-Hour Days | Every Other Weekend | First to Float
Hours: 40 hours/week | 13-week contract (91 days)
Setting: Acute Care | Hospital

Position Overview:

The RN Care Coordinator supports hospital case management operations through discharge planning, utilization review, and coordination of patient care across the continuum. The nurse ensures efficient, high-quality patient outcomes while adhering to hospital policies and regulatory requirements.

Key Responsibilities:

• Perform comprehensive patient assessments to identify discharge and care coordination needs.
• Collaborate with physicians, interdisciplinary teams, patients, and families to develop and execute care plans.
• Conduct utilization reviews and ensure medical necessity and documentation compliance.
• Manage patient throughput and timely discharges using systems such as Midas, InterCall, Cerner, and Curaspan.
• Float to adult units as required (excludes women’s health if not experienced).
• Maintain accurate, timely documentation of all case management activities.

Requirements:

• Experience: Minimum 3 years of hospital/acute care coordination experience.
• Technical Skills: Proficiency in Midas, InterCall, Cerner, Curaspan, and Microsoft Office .
• Certifications: BLS (Required) .
• Licensure: Active RN license (CA or Compact).
• Preferred: Prior Dignity/CommonSpirit acute care experience (mandatory for consideration).
• Education: BSN not required.
