Wellness Nurse - RN or LPN

We are looking for a Wellness Nurse RN- Registered Nurse or LPN - Licensed Practical Nurse to join our dynamic team.

• Per Diem -- NEED TO BE ABLE TO WORK SOME 7:00AM - 3:00 PM SHIFTS.
• Pay Range for RN: $27k to $38 per hour; for LPN: $23 to $34 per hour.
• Very stable leadership and low turnover.

Registered Nurse or LPN - Licensed Practical Nurse Wellness Nurse Qualifications:

• MA RN or LPN License.
• 1 year of Long-Term Care experience.

Registered Nurse or LPN - Licensed Practical Nurse Wellness Nurse Job Summary:

• Implement and coordinate resident care in collaboration with physician and nursing staff.
• Document all resident care information.
• Provide functional direction and assistance to LPNs and CNA’s.
• Complete new staff orientation as assigned.
• Direct resident care to include making round, answering call lights, administer medications.
• Complete monthly/quarterly assessments and documentation.
• Attend care plan meetings.
• Communicate effectively with the residents, families, visitors, members of the health care team, and other disciplines.
• Participate in screening potential admissions.
