The Disaster Case Manager will provide key support as Survivors of the LA Wildfires (Eaton, Palisades, Sunset, Hurst) rebuild their lives. The Case Manager will provide services to clients who are displaced or lost their homes. Services include developing short term and long-term housing plans, navigating FEMA and insurance claims, addressing financial gaps needed for stability and connecting with other community and government resources. Services are provided at JFS sites and in the community. This position is time limited and is estimated to end after two years .

QUALIFICATIONS:

  • BA Degree in social work or related health or social services field.
  • Bi-lingual Spanish, Armenian a plus
  • Current unrestricted CA Driver’s License and proof of auto insurance.
  • Demonstrated skills, interest and sensitivity in working with community members who have experienced trauma and working/traveling in the field
  • Knowledge of community and government resources and the ability to advocate on behalf of clients
  • Experience providing field-based case management services a plus
  • Bi-lingual (English/Spanish) a plus
  • Excellent communication skills both written and oral and ability to utilize telehealth platforms
  • Excellent computer skills using Microsoft Word, Outlook and excel programs
  • Candidates must have interest and demonstrated ability to work in an interdisciplinary community setting
  • Be culturally sensitive, able to work with a multicultural client population and work in a non-judgmental manner
  • Foster positive growth/outcomes as part of a collaborative team
  • Able to maintain professional boundaries
  • Must have strong casework judgement and fiscal management skills
  • Must be able to pass a criminal background check (Live Scan fingerprinting) and drug screen prior to beginning employment.

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Provide short and long-term case management support to community members impacted by the wildfires in temporary housing and/or in search of permanent housing.
  • Outreach, engagement and rapport building with those impacted by the disaster including vulnerable and other special needs populations who may be socially or geographically isolated, of limited English proficiency and individuals with disabilities, children, elderly and any other who likely has unmet disaster-related needs.
  • For registrants or family members identified as having an Access or Functional Need (AFN) such as equipment, products, or services, ensure case managers assist early on in the process to restore the equipment, products or services as needed.
  • Assessment of unmet short- and long-term recovery needs for wildfire survivor clients.
  • Develop short- and long-term recovery goals and implement plans through case management support to clients as they work towards recovery goals.
  • Provide information and referral for specific community services and supports to address these needs.
  • Connecting clients with immediate resources for food, shelter, medical and emotional support.
  • Provide guidance and advocacy to clients re: insurance claims, FEMA and legal resources.
  • Assist with applications for funds available through government and other community foundations to address unmet financial needs during rebuilding or relocation process.
  • Maintain proper and timely documentation/records of services provided in the electronic health record - Efforts to Outcomes (ETO), to monitor and track the progress of clients from entry to exit.
  • Collaborate with all DCMP team members for cohesive service delivery and reaching plan goals.
  • Provide community outreach; develop and maintain relationships with other community providers to facilitate coordinated services for clients.
  • Report consistently and timely all work performed, population needs met, services provided, as related to fulfillment of project goals and milestones to include information regarding AFN equipment, products, or services provided.
  • Additional related responsibilities as required.
